I'd say the main (4th) form, but that seems a little too obvious, so I'm gonna go with the 2nd "Kamata-Kun" form because he looks like a googly-eyed armless frilled shark/moray eel/rubber chicken hybrid and is just too weird and goofy not to be loved.

I like Kamata-kun (although Shinagawa-kun doesn't get as much credit as he deserves either), 4th form is obvious, but I actually also really like the 5th form human things
